The bait in that photo is a 5-inch Berkley Powerbait Hollow Belly swimbait.
However, there are about 15 different baits on the market that are exactly the same - and I speak from experience, because I own them all.
The original soft-body swimbait these types of swimbaits are patterned after are Basstrix baits out of California. However, Basstrix baits are hard to find and they carry a pretty hefty price tag.
The Strike King Shadalicious is another identical bait. One good thing about the Shadalicious is that the plastic seems to be a little tougher, which is better for saltwater applications:

The Mann's Hard Nose is another bait I've been using a lot lately. The head of this bait is solid plastic, which means it stays on the hook better and doesn't slide or tear when you set the hook. Another big plus is that it comes packaged with a weighted hook:

Speaking of hooks, you'll need to pay special attention to the types of hooks you use with these big baits. If you're fishing sparse cover where you're not likely to hang up you can use a simple jighead - the longer the shank the better.
However, if you're fishing heavy cover, a hook like this one is ideal:

I'd go with the biggest one you can buy (5/0 in this case).
